الإجابة: D. Acute Interstitial Nephritis (AIN)

✓ مطابق للمصدر Acute Kidney Injury [Notes].pdf · p. 5

AKI after starting a new drug (e.g. a beta-lactam antibiotic) with fever, rash, arthralgia, pyuria and WBC casts is acute interstitial nephritis (an allergic, drug-induced tubulo-interstitial injury; urine eosinophils may be seen). Treatment is stopping the offending drug.

  • ATN gives muddy brown granular casts, not WBC casts or a rash.
  • Minimal change disease and Alport syndrome are glomerular and don't fit.
