HEME-01 Microcytic Anemia [Notes].pdf · p. 9
Anaemia in CKD with TSAT 15% (<20%) is iron-deficient. Iron must be repleted before starting erythropoietin: EPO is given only once iron deficiency is corrected and works poorly without it. That rules out starting EPO (A) first, and transfusion (B) is not needed at Hb 9 without instability.
- The HEME notes say CKD anaemia may require IV iron before EPO, which supports IV iron (D).
- Davidson reserves IV iron mainly for oral intolerance or poor adherence, so oral iron (C) is a defensible alternative in non-dialysis CKD.
