A 35-year-old man presents with a 4-day history of severe gastroenteritis associated with persistent vomiting and diarrhea. He appears dehydrated with dry mucous membranes and hypotension. Laboratory investigations show an acute rise in serum creatinine consistent with acute kidney injury (AKI). What is the most likely type of AKI in this patient?

الإجابة: A. Pre-renal AKI

✓ مطابق للمصدر Acute Kidney Injury [Notes].pdf · p. 3

Vomiting and diarrhoea cause hypovolaemia, which lowers effective arterial blood volume and renal perfusion. That is pre-renal AKI: typically a raised BUN:creatinine ratio and FENa <1%, and it reverses with fluids.

  • Intrinsic AKI (ATN, AIN, GN) needs damage to the kidney itself. Prolonged prerenal hypoperfusion can progress to ATN, but the picture here is pre-renal.
  • Post-renal AKI needs obstruction.
  • CKD is not acute.
