Chronic renal failure is the most frequent cause of secondary hyperparathyroidism. Chronic hypocalcemia stimulates all four glands, which become hyperplastic. If this goes on long enough it becomes autonomous (tertiary HPT). A single adenoma (C) is the typical cause of primary HPT, not of renal disease. Carcinoma is rare.
