A discrete nodule on the isotope scan of a Graves' gland is usually a cold (non-functioning) nodule. Cold nodules raise suspicion for malignancy. Tissue diagnosis is by fine-needle aspiration cytology, guided by ultrasound features. CT, MRI, or ultrasound for lymph nodes alone do not tell whether the nodule is benign or malignant.
