Stridor immediately after extubation following total thyroidectomy is the classic sign of bilateral recurrent laryngeal nerve palsy: both cords lie near the midline and obstruct the airway. Superior laryngeal nerve injury only weakens the voice and causes loss of high pitch. Unilateral RLN injury causes hoarseness.
