A 57-year- old male patient presented to the clinic with painful bleeding per rectum. He was complaining of constipation for many days prior to this presentation. On examination, it was very difficult to examine the anus and to perform digital rectal examination but there was no obvious mass. What would be your initial plan of management?

Painful bright bleeding after constipation, with an anus too painful to examine (sphincter spasm) and no mass, is an acute anal fissure. Initial management is conservative: analgesia, laxatives/fibre and sitz baths, then follow-up. Sphincterotomy (D) is kept for chronic or refractory fissures. Hemorrhoidectomy, angiography and urgent colonoscopy are not first steps here; elective endoscopy can follow once the pain settles.
