Second-degree hemorrhoids without active bleeding are managed conservatively first: diet and lifestyle change, fibre and laxatives (also important for a heavy labourer who strains). Banding or sclerotherapy (B, C) is used only if symptoms do not respond. Hemorrhoidectomy (A) is for grade III/IV or failed treatment, and Botox is not a hemorrhoid treatment.
