A crush injury with a markedly raised CK, rising creatinine/BUN and urine that is positive for blood but shows no RBCs (myoglobinuria) is rhabdomyolysis causing pigment-induced ATN.
- Prerenal azotemia fits the hypotension but does not explain the very high CK or the heme-positive urine without RBCs.
- AIN (drug, eosinophils) and postrenal obstruction have no support in the stem.
- Treatment: aggressive IV fluids.
