With GFR < 60, patients should avoid nephrotoxins; NSAIDs (ibuprofen) reduce renal blood flow by constricting the afferent arteriole and can precipitate AKI or accelerate CKD.
- Lisinopril is the tempting distractor, but ACE inhibitors are recommended in diabetic nephropathy/proteinuric CKD because they slow progression (just monitor creatinine and K+).
- Folic acid and oral iron are safe supportive treatments.
