الإجابة: A. FNA
An incidental >1 cm nodule with indeterminate/suspicious ultrasound features needs FNA. Ultrasound grade (TI-RADS) decides on FNA, and FNA is the only test that reliably separates benign from malignant nodules. Serial ultrasound or no action fits nodules with benign-looking features. Hemithyroidectomy before cytology is premature; surgery depends on the Bethesda result.

B
ليش غلط
Periodic ultrasound is for a nodule whose FNA came back benign; this one is indeterminate on ultrasound and has not had FNA.

C
ليش غلط
Surgery is indicated when FNA is suspicious or malignant; no cytology has been done yet, so hemithyroidectomy is premature.

D
ليش غلط
An ultrasound with indeterminate features cannot be ignored: nodules may be malignant, and FNA is what separates benign from malignant.
