A 64-year-old woman has palpitations, anxiety and weight loss after flu-like symptoms; she had AF that reverted with β-blocker. TSH 0.05, FT4 45, TRAb and TPO negative. Thyroid normal size and tender; uptake <2% at 24 h. No amiodarone. What is the diagnosis?

الإجابة: A. Thyroiditis (subacute)
Thyrotoxicosis after a flu-like illness with a tender thyroid and very low radioiodine uptake is subacute (de Quervain) thyroiditis: hormone leaks from the inflamed gland, which cannot trap iodine. Graves gives a high, diffuse uptake with positive TRAb. Drug-induced thyroiditis is excluded because she has no amiodarone exposure. Thyroid cancer does not cause this picture.

-
A
ليش صح
Subacute thyroiditis: damaged follicular cells cannot trap iodine, so radioiodine uptake is low, with TSH suppressed by raised T4/T3.

B
ليش غلط
Graves disease gives an enlarged gland with diffusely increased uptake; her uptake is below 2% and TRAb is negative.

C
ليش غلط
Drug-induced thyroiditis is tied to drugs such as amiodarone or lithium; none is reported here.

D
ليش غلط
Thyroid cancer suggests a fixed, firm or irregular nodule; she has a normal-sized, tender gland after a flu-like illness.
