الإجابة: B. Transthoracic echocardiography (TTE)

✓ مطابق للمصدر Step Up To Medicine 6th (Valvular Heart Disease).pdf · p. 11

Suspected endocarditis (IV drug use, fever, new murmur, MSSA bacteraemia) needs echocardiography. The sources say most patients should get TTE as the initial screening test. TEE is more sensitive and is used next if TTE is negative or unclear, or for prosthetic valves and suspected complications. It is not the first step. Oral antibiotics and steroids are wrong: IE needs prolonged IV antibiotics.

ليش كل خيار صح أو غلط

  • A ليش غلط

Endocarditis needs parenteral antibiotics for extended periods (up to 6 weeks), guided by cultures. Oral antibiotics with follow-up in two weeks is inadequate and does not confirm the diagnosis.

  • B ليش صح

TTE is the initial screening echo for most patients with suspected endocarditis; here IV drug use, fever, a new murmur and MSSA bacteremia call for echocardiography.

  • C ليش غلط

TEE is better than TTE for mitral valve pathology and small aortic vegetations, but TTE is still the initial screening test for most patients.
