A 52-year-old man with known mitral valve prolapse has 2 weeks of fever, malaise and night sweats. He had a tooth extraction shortly before the symptoms started. Temperature 38.7°C. He has splinter haemorrhages and a painless erythematous lesion on the palm. Blood culture: Gram-positive cocci in chains. Which organism is most likely?

الإجابة: D. Viridans group streptococci
Subacute endocarditis on an abnormal native valve (MVP) after a dental extraction, with Gram-positive cocci in chains, is classic for viridans group streptococci: they are normal mouth flora and the most common cause of native-valve IE. Staph aureus grows in clusters and causes acute IE, typically in IV drug users. Enterococcus follows GI/GU procedures. Strep pyogenes is not a typical IE organism. The painless palm lesion is a Janeway lesion.
