A 42-year-old man has had breathlessness for 2 months, worse lying down. No cough or fever. He is vitally stable. There is a grade 3/6 systolic murmur radiating to the left axilla, which gets louder with expiration and squatting. What is the best next step to establish the diagnosis?

الإجابة: D. Echocardiogram
A systolic murmur radiating to the left axilla with orthopnoea suggests mitral regurgitation. All valvular heart disease is confirmed with echocardiography (TTE), which shows the valve lesion, how severe it is, and LV size and function. A chest X-ray only shows congestion or chamber enlargement and cannot make the diagnosis. Stress testing and cardiac MRI are not the first test for a valve lesion.

In mitral regurgitation a chest X-ray only shows pulmonary congestion or left atrial/ventricular enlargement; it does not establish the valve diagnosis.

A treadmill stress test looks for coronary plaque: exercise raises O2 demand and unmasks ischemia (ECG, perfusion, wall motion). It is not the test for a valve lesion.

Cardiac MRI is a specialised test for cardiomyopathies and infiltrative disease (e.g., amyloidosis), not the first test for a valve murmur.

Echocardiography is the modality of choice for the initial assessment of any suspected valvular abnormality, including mitral regurgitation with its axillary-radiating pansystolic murmur.
