الإجابة: E. exogenous cortisol
Cushingoid features with low ACTH are ACTH-independent (adrenal tumour or exogenous steroids). Low DHEA shows suppressed adrenal androgens, and a normal urinary cortisol argues against an adrenal adenoma/carcinoma, which would make urinary free cortisol high. Cushing disease and ectopic ACTH both have high ACTH.

ليش غلط
Adrenal adenomas are ACTH-independent tumours that produce excess cortisol themselves, which does not fit the normal urine cortisol seen here.

B
ليش غلط
Adrenal carcinomas are ACTH-independent tumours that produce excess cortisol themselves, which does not fit the normal urine cortisol seen here.

C
ليش غلط
Cushing disease is an ACTH-secreting pituitary adenoma, so ACTH would be raised; here ACTH is low.

D
ليش غلط
Ectopic ACTH syndrome is an ACTH-secreting tumour outside the pituitary driving cortisol, so ACTH would be raised; here ACTH is low.

E
ليش صح
Exogenous steroids suppress adrenal androgen production (low DHEA) while causing Cushingoid features; it is also the most common cause of hypercortisolism.
