A 37-year-old woman has fatigue, muscle weakness, and weight gain. She has no past medical history and is not taking any medications. On examination, her BP is 164/92 mmHg and HR 84/min. She has multiple skin bruises, facial fullness, and truncal obesity with red "stretch marks." Muscle strength in the proximal muscles is 4+/5 and reflexes are normal. الإجابة: B. Overnight dexamethasone suppression test
Bruising, moon face, truncal obesity, purple striae, proximal weakness and hypertension suggest Cushing syndrome. The overnight (1 mg) low-dose dexamethasone suppression test is the initial screening test. Imaging (abdominal CT, brain MRI) comes only after biochemical confirmation, and EMG does not diagnose hypercortisolism.
ليش كل خيار صح أو غلط
-
A
ليش غلط
Imaging for the cause comes after Cushing syndrome is established and ACTH is measured (low ACTH suggests an adrenal cause); biochemical screening comes first.

D
ليش غلط
Imaging to find the cause is not the initial test: Cushing syndrome is first established biochemically, then ACTH is measured.
