A painless, smooth midline neck lump that moves with tongue protrusion is a thyroglossal cyst, because it is attached to the remnant of the thyroglossal duct. A dermoid cyst is midline but does not move with the tongue. A branchial cyst is lateral, at the anterior border of the sternocleidomastoid. A thyroid isthmus mass moves with swallowing, not tongue protrusion.
