Thyroid and parathyroid surgery (1).pdf · p. 11
A patient with long-standing chronic renal failure who becomes hypercalcemic has tertiary hyperparathyroidism. Long-standing secondary disease causes reactive parathyroid hyperplasia that becomes autonomous and keeps secreting PTH. A solitary adenoma is the usual cause of primary hyperparathyroidism, carcinoma is rare, and atrophy would not raise calcium.
