Breathing difficulty with a large anterior neck swelling hours after thyroidectomy is a compressive hematoma. The sources say to open the wound immediately at the bedside to decompress the trachea. Intubation attempts are difficult while the hematoma is compressing the airway, and imaging or a CBC only delays decompression; tension pneumothorax does not fit the picture.
