After appendectomy for a perforated (contaminated) appendix, the listed complications are wound infection and intra-abdominal (pelvic) abscess. Fever on day 5 fits the wound/abscess window. Right iliac fossa pain with diarrhea suggests a deep pelvic collection irritating the rectum.
- Mesenteric lymphadenitis is a pre-operative mimic of appendicitis, not a post-op complication.
- Diverticulitis and pyelonephritis do not fit this setting.
