U waves (a small wave after the T wave, best seen in V2-V3) are a classic ECG sign of moderate to severe hypokalemia, together with flat T waves, ST depression and QT prolongation. Hypercalcemia shortens the QT interval, and hypernatremia has no typical ECG change.
