Fever + RUQ pain in an ill patient with a cholestatic enzyme pattern (ALP and GGT far higher than the transaminases) points to acute cholangitis - infected bile in an obstructed duct, diagnosed by raised cholestatic enzymes.
- Acetaminophen toxicity and viral hepatitis give a hepatocellular pattern, where AST/ALT dominate (often in the thousands in acute injury), not a picture led by ALP/GGT.
