When bowel perforation is suspected, the first quick test is an erect chest X-ray, which shows free air under the diaphragm (pneumoperitoneum). Ultrasound is poor for free air. A barium enema is dangerous when perforation is possible. Laparoscopy is invasive and not the first confirmatory test.
