The test of choice for suspected acute diverticulitis is CT of the abdomen and pelvis with IV contrast. It confirms the diagnosis and shows how severe it is (abscess, perforation). Colonoscopy is avoided in the acute phase because it can cause perforation. It is done 2–6 weeks later. A plain abdominal series only rules out free air or ileus.
