A 46-year-old male patient attended the ER complaining of abdominal pain and vomiting of 1-day duration. He underwent an appendectomy at the age of 18 years. Abdominal examination revealed tenderness over the appendectomy scar with a resonant percussion note, no rebound phenomenon and no skin changes and vital signs reasonably stable. All his blood tests were normal. A plain abdominal film showed multiple dilated small bowel loops. Which of the following is correct regarding this condition?

الإجابة: C. Dilated bowel loops are highly suggestive of bowel obstruction (together with the clinical picture)
This is the same case as D-121. Dilated small bowel loops, interpreted with the clinical picture, strongly suggest obstruction.
- A is false: adhesive SBO often resolves with drip-and-suck.
- B is false: fever is not a cardinal feature (those are pain, distension, vomiting and constipation).
- D is false: adhesions, not tumors, are the commonest cause of SBO in adults.
