An elderly woman with left iliac fossa pain, tenderness and guarding, fever and tachycardia most likely has acute diverticulitis, possibly complicated. The investigation of choice is a contrast CT of the abdomen and pelvis, which confirms the diagnosis and shows complications such as abscess or perforation. Colonoscopy and barium enema are not done in the acute inflamed phase, and laparoscopy is not a first-line diagnostic test.
