Starting an ACE inhibitor causes an expected early fall in GFR (efferent arteriole dilation). Treatment is continued as long as the fall in GFR is ≤ 25% and not progressive; here creatinine rose 1.5 → 1.8 (~20%), so continue lisinopril and recheck renal function.
- Stopping it (B, C) would forfeit its renoprotective effect; stop only if the GFR drop is > 25% or progressive.
- Adding valsartan (dual ACEi + ARB) is not recommended.
