A young woman with cerebellar/brainstem signs (nystagmus, dysarthria, ataxia) and a normal CT needs an MRI with and without (gadolinium) contrast.
- CT visualises the posterior fossa poorly, so a normal CT does not exclude a lesion there.
- In a young woman, demyelination (MS) is a key concern: MRI is the most sensitive test, and gadolinium shows active lesions.
- LP is supportive only, and CTA/CTV is for vascular or venous sinus questions.
