Anaemia with thrombocytopenia, high LDH, a negative Coombs test, renal impairment and confusion suggests a thrombotic microangiopathy (TTP/HUS). The key investigation is the peripheral blood film, which shows schistocytes and confirms microangiopathic haemolysis. ADAMTS13 then separates TTP from HUS. Bone marrow biopsy, blood/stool cultures and abdominal X-ray don't establish MAHA.
