Continuous inflammation starting in the rectum and extending proximally (here to the splenic flexure), with a normal terminal ileum, and histology showing crypt abscesses and crypt architectural distortion without granulomas is ulcerative colitis. The knee/ankle arthralgia is an extraintestinal manifestation. Crohn's colitis would show skip lesions, transmural inflammation, often ileal involvement and granulomas; ischaemic colitis occurs in older vascular patients with acute pain; microscopic colitis causes non-bloody watery diarrhoea with a normal-looking mucosa.
