Fatigable proximal (shoulder) weakness, ptosis worsening on sustained upgaze and evening diplopia = myasthenia gravis. Of the options, the diagnostic test is repetitive nerve stimulation, which shows a characteristic decremental response; antibodies (anti-AChR/MuSK) are the other key test but are not offered. Plain EMG is less specific, CSF is for GBS/infection, and vitamin D is irrelevant.
