A DAT positive for IgG in a patient with SLE means warm autoimmune haemolytic anaemia. SLE is a classic secondary cause. The IgG-coated RBCs are destroyed by phagocytes in the reticuloendothelial system, so the haemolysis is extravascular.
- Cold AIHA is IgM/C3d-mediated.
- Complement-mediated intravascular haemolysis is typical of IgM (cold) antibodies, not IgG.
- "Destruction of RBCs" is too non-specific.
