Ptosis + a dilated pupil + inability to move the eye medially and upward = left oculomotor (CN III) palsy. CN III supplies levator palpebrae, superior/medial/inferior rectus, inferior oblique and (via Edinger-Westphal fibres) the pupillary constrictor.
- Trochlear (CN IV) only moves the eye down and out; abducens (CN VI) only abducts; the optic nerve does not move the eye or lid.
