Raised JVP, bilateral crackles and leg oedema in ESRD = volume overload / pulmonary oedema. Immediate treatment is a loop diuretic (furosemide), titrated to urine output. If there is no response, the patient needs dialysis.
- Beta-blockers are not acute therapy for decompensated overload.
- Digoxin ("Dioxin" in the stem) accumulates in renal failure and is not first-line.
- Lidocaine is an antiarrhythmic and irrelevant here.
