An acute ischaemic stroke (no bleed on CT) with a proximal MCA thrombus on CTA is a large vessel occlusion. That is the indication for mechanical thrombectomy, a catheter/stent-retriever clot removal (with or without IV thrombolysis). Heparin infusion is not acute reperfusion therapy. Observation or repeating the CT wastes salvageable penumbra.
