الإجابة: B. β-hCG, TSH, FT4, LH, FSH, prolactin
Amenorrhoea + persistent galactorrhoea suggests hyperprolactinaemia. The source work-up starts with excluding physiological causes (pregnancy) and hypothyroidism, then checking prolactin — i.e. β-hCG, TSH/FT4, prolactin (with LH/FSH for the amenorrhoea). MRI comes after the biochemistry confirms hyperprolactinaemia without another cause, so A is premature.

Galactorrhea with amenorrhea is the female presentation of hyperprolactinemia, whose work-up is physiological causes, hypothyroidism/drugs and prolactin, not cortisol testing.

Pelvic US and a progesterone withdrawal test are not in the listed work-up for hyperprolactinemia (physiological causes, hypothyroidism and drugs, prolactin level, MRI), which galactorrhea with amenorrhea suggests.
