الإجابة: D. Linezolid
Pneumonia from day 3 of admission is HAP, and the sputum grows MRSA, so the drug must have MRSA activity: linezolid (or vancomycin). Cefepime, ciprofloxacin and amoxicillin-clavulanate are the anti-Gram-negative/standard partners in HAP regimens, not MRSA cover.

A
ليش غلط
Oral amoxicillin-clavulanate is used for sinusitis, otitis media and dental infections. HAP needs IV anti-pseudomonal and anti-MRSA cover.

B
ليش غلط
Ciprofloxacin covers gram-negatives including Pseudomonas, but only moxifloxacin and levofloxacin have some gram-positive activity, so it is not MRSA therapy.

C
ليش غلط
Cefepime is an antipseudomonal beta-lactam used as the Pseudomonas partner in HAP regimens; it is not the MRSA agent (that is linezolid or vancomycin).

D
ليش صح
Linezolid is the drug of choice in MRSA pneumonia, which is what this HAP is (the sputum grows MRSA).
