الإجابة: A. Chest X-ray
Fever, productive cough, pleuritic pain and reduced breath sounds suggest pneumonia ± parapneumonic effusion; the confirmatory first test is a chest X-ray. CT is reserved for an inconclusive CXR or recurrent pneumonia, and thoracentesis follows only once an effusion is shown. TTE does not diagnose this.
