الإجابة: C. TTE
An IV drug user with fever, a new pansystolic murmur, Osler nodes and positive blood cultures has suspected infective endocarditis; the next diagnostic test is echocardiography, starting with TTE as the initial screening test. TEE is more sensitive and is used if TTE is non-diagnostic; CXR and ECG do not confirm IE.

In endocarditis the ECG may only show a long PR interval; the imaging that shows vegetations is echocardiography (TTE may show vegetations over 2 mm).

C

TTE is the initial screening echocardiogram for most patients with suspected endocarditis (IV drug use, fever, murmur, positive cultures).

D
ليش غلط
TEE is better than TTE for mitral pathology and small aortic vegetations, but TTE remains the initial screening test for most patients.
