الإجابة: D. Refer for aortic valve replacement
Severe AS (valve area 0.7 cm²) that is symptomatic (exertional dyspnoea, heart failure signs) is an indication for aortic valve replacement; medical therapy has only a limited role. Diuretics or ACE inhibitors just treat symptoms, and Holter with follow-up is not appropriate for symptomatic severe AS.

Conservative follow-up is for asymptomatic or mild-to-moderate AS; symptomatic or severe AS goes to valve replacement or repair. This patient has symptomatic severe AS.

Management of AS is straightforward: symptomatic AS needs valve replacement (surgical or TAVR); diuretics only treat symptoms.

Management of AS is straightforward: symptomatic AS needs valve replacement (surgical or TAVR); an ACE inhibitor does not fix the obstruction.
