الإجابة: C. Thyroid ultrasound
For a thyroid nodule with a normal TSH, the next step is thyroid ultrasound. It describes the nodule (TI-RADS) and decides whether FNA is needed. A thyroid scan is kept for a low TSH (suspected hot nodule) or an indeterminate FNA. T3 adds nothing when TSH/T4 are normal. Resection before US and cytology is premature.

-
A
ليش غلط
A thyroid scan is first-line for uncertain thyrotoxicosis; here TSH and T4 are normal, so the nodule is first assessed by ultrasound.

B
ليش غلط
Thyroid function is already assessed: TSH is the initial screening test and is normal along with T4, so the next step is nodule imaging rather than another hormone level.

C
ليش صح
The initial work-up of a thyroid nodule is history, examination, ultrasound of the neck and TSH; the ultrasound then decides on FNA.

D
ليش غلط
Surgery is premature: cancer is found in only 5% to 10% of investigated nodules, so ultrasound, TSH and FNA come first.
