الإجابة: B. Start levothyroxine and recheck TSH and lipids in six weeks
High TSH with low T4 is overt primary hypothyroidism, which is a recognised cause of secondary hyperlipidaemia (less LDL is taken up by the liver). Treat the cause: start levothyroxine and re-check TSH (and lipids) after about 4–6 weeks. Lipids often improve without other treatment. A statin alone or diet alone ignores the thyroid cause. Uncomplicated primary hypothyroidism does not need endocrinology referral.
ليش كل خيار صح أو غلط
-
A
ليش غلط
Diet alone misses the cause: low thyroid hormone reduces LDL uptake by the liver, so the high lipids need thyroid hormone replacement.

B
ليش صح
High TSH with low T4 is primary hypothyroidism; treatment is levothyroxine, re-evaluated after 4 to 6 weeks.

C
ليش غلط
A statin alone treats the cholesterol but not its cause: low T4 means less LDL uptake by the liver, so replacing thyroid hormone comes first.

D
ليش غلط
Endocrinology referral is listed for severe new cases, pregnancy or secondary hypothyroidism; this is uncomplicated primary hypothyroidism, treated with levothyroxine.
