Has body temperature of 39.3°C, multiple pink, painful nodules on the palmar surface of both hands and grade 3 pansystolic murmur on fourth left parasternal area. Three sets of blood culture are positive for Methicillin-sensitive Staphylococcus aureus (MSSA). What is the drug of choice for this patient?

الإجابة: C. Intravenous Oxacillin
An IV drug user with fever, painful finger-pulp nodules (Osler nodes) and a tricuspid-area pansystolic murmur has infective endocarditis. Blood cultures grow MSSA. Anti-staphylococcal penicillins (oxacillin, nafcillin, flucloxacillin) are the drug of choice for MSSA bacteraemia. Clindamycin is not adequate for endovascular infection. Gentamicin is not used alone. Aztreonam covers only Gram-negatives.

Clindamycin is listed for anaerobic abdominal or genital infection and staphylococcal infection of joint, bone, teeth and diabetic foot. For MSSA bacteremia the drug of choice is an anti-staph penicillin.
