Basal inspiratory crackles with parenchymal disease on HRCT means interstitial lung disease, a restrictive disorder. Reduced lung expansion makes the FVC markedly reduced, while FEV1 is only mildly reduced, so the FEV1/FVC ratio stays normal or high (>70%) and lung volumes (TLC, RV) fall. A low FEV1/FVC ratio, markedly low FEV1 and hyperinflation are features of obstructive disease.
