الإجابة: B. Cystic fibrosis
A young man with lifelong productive cough and bronchiectasis plus pancreatic involvement (malabsorptive diarrhoea, poor weight gain, diabetes) and infertility fits cystic fibrosis: the CFTR chloride-channel defect makes thick secretions in the respiratory tract, exocrine pancreas, intestines and genitourinary tract. CF is the most common cause of bronchiectasis. COPD, ILD and alveolar haemorrhage do not explain the GI and reproductive features.
