الإجابة: C. IVC filter
Recent variceal upper GI bleed is an active/very recent bleed, i.e. a contraindication to anticoagulation. In DVT with a contraindication to anticoagulation, place an IVC filter to prevent PE, and start anticoagulation once bleeding risk falls. LMWH and IV heparin both carry bleeding risk here.

A
ليش غلط
LMWH is an anticoagulant, and active bleeding is an absolute contraindication to anticoagulation; after a recent variceal bleed the answer is an IVC filter until bleeding risk falls.

B
ليش غلط
IV heparin is still an anticoagulant; with an absolute contraindication such as active bleeding, an IVC filter is used first and anticoagulation starts once bleeding risk is low.

C
ليش صح
An IVC filter is used in acute DVT/PE when anticoagulation is absolutely contraindicated (e.g. active bleeding); anticoagulation starts later once bleeding risk is low.
