  • إجابة: B. Parapneumonic effusion
  • إجابة ثانية: A. Empyema

In the main lecture (BTS classification), an exudate after pneumonia with pH <7.2 but no frank pus or positive Gram stain/culture is a complicated parapneumonic effusion; empyema means pus in the pleural space. Step Up also says pH <7.2 can be either. But the resident lecture and a student summary label pH <7.2 / glucose <60 / LDH >1000 as empyema, which is the old bank key (A). Either way, the effusion needs chest tube drainage.

ليش كل خيار صح أو غلط

  • A ليش صح

Supported by the Pulmonology summary, which labels pH below 7.2, glucose below 60, LDH above 1000 and a positive culture as suggestive of empyema.

  • B ليش صح

Supported by the BTS classification in the Pleural effusions slides: pH below 7.2, LDH above 1000 and low glucose with clear or turbid fluid and a negative Gram stain/culture is a complicated parapneumonic effusion; the slides reserve empyema for frank pus or a positive Gram stain/culture.
