الإجابة: D. Vasovagal syncope
A young person fainting at the sight of blood (emotional trigger), with bradycardia during the episode and quick recovery on leg elevation, has vasovagal (reflex/neurocardiogenic) syncope - the most common cause of syncope. A paradoxical surge in vagal tone causes inappropriate bradycardia and vasodilation. Complete heart block would not be triggered by emotion in a healthy 21-year-old, and TIA/drop attacks do not fit.

A
ليش غلط
"Drop attacks" are a vertebrobasilar TIA phenomenon from severe cerebrovascular disease, a rare cause of syncope, not a faint at the sight of blood.

B
ليش غلط
A TIA is a rare cause of syncope, and vertebrobasilar insufficiency always comes with other deficits as well.

C
ليش غلط
Cardiac syncope (including AV block) is usually sudden without prodrome; this faint had an emotional trigger and reversed when her legs were raised, the vasovagal pattern.

D
ليش صح
Vasovagal syncope is triggered by emotional stress, pain or fear; a vagal surge causes inappropriate bradycardia and vasodilation, and it reverses with legs elevated.
