A 67-year-old woman with hypertension is complaining of shortness of breath. Her blood pressure is 140/40 mmHg and she has an early diastolic murmur along with the left third intercostal space. The murmur is best heard on sitting forward and deep inspiration What type of pulse can be associated with this valvular heart disease?

الإجابة: B. Collapsing pulse
Early diastolic murmur at the left sternal edge, louder sitting forward, with a wide pulse pressure (140/40) = aortic regurgitation. AR gives a collapsing (water-hammer / Corrigan) pulse: rapid upstroke that collapses suddenly. Pulsus parvus et tardus belongs to aortic stenosis, and pulsus paradoxus to tamponade.

A
ليش غلط
Pulsus paradoxus (inspiratory fall in systolic BP above 10 mmHg) is listed with cardiac tamponade, severe airway obstruction, PE and constrictive pericarditis, not aortic regurgitation.

B
ليش صح
Aortic regurgitation (early diastolic murmur at the left sternal edge, wide pulse pressure) is associated with a water-hammer (collapsing) pulse.

C
ليش غلط
Pulsus parvus et tardus is a sign of aortic stenosis (exertional chest pain, syncope, ejection murmur), not of aortic regurgitation.

D
ليش غلط
Pulsus alternans appears in the heart-failure notes among the signs of failing ventricles (gallop, raised JVP), not as a feature of aortic regurgitation.
